How to Send a Referral

We accept orders in two ways:

  • Through our Provider Portal (preferred), place orders, review your patients' images and reports, and see when your patient has scheduled their appointment, all in one place. No more calls to the front desk wondering if they're on the books.
    Access the Portal below or click the button at the bottom of the page.
  • By fax. Fax numbers vary by location. Visit our Locations page for the number closest to you.

New to the portal? Visit the Provider Portal sign-in page and select "Request Access" beneath the login fields to request a new account. For technical assistance, contact our IT team at (573) 817-1224.


What to Include for Prior Authorization

Musculoskeletal

✅ Previous imaging (usually X-ray)

✅ Documented 6 weeks of conservative treatment (usually physical therapy, but medications tried, provider-guided home exercises, etc.) 

✅ Results of physical exam within the last 3 months

Abdominal / Pelvic

✅ Previous imaging (usually US)

✅ Results of any relevant lab work

✅ Rule out

✅ Any treatments tried

✅ Results of physical exam within the last 3 months 

Urinary Stones (CT)

✅ Ultrasound results (required by some plans, including many BCBS/Anthem plans)

✅ Date of any previous episodes, how many instances within the past year

✅ Composition of previous stones, if known 

✅ Location of abdominal pain

✅ If the patient is experiencing hematuria

✅ Results of any relevant lab work 

Pain Intervention

✅ Dates of any previous injections

✅ Most recent imaging results (usually need to be within 1 year)

✅ Outcome of previous injections, including degree and duration of relief

✅ Does pain interfere with work, school, exercise, or leisure? 

MS Protocol

✅ Any clinical changes since previous imaging?

✅ Is MS already diagnosed or suspected? 


ALL exams will require the most recent office note and typically must be within 3 months. 

 
Some BCBS/Anthem plans require authorization for arterial ultrasounds. 

If your office has already attempted authorization at another facility, please let us know — we typically cannot change the facility ourselves, so it would need to be withdrawn before we can submit a new one.


Report Turnaround

Most reports are read and returned within 24 hours. We quote 24–48 business hours to account for volume, but we take fast turnaround seriously.

Important Notice: Supply Chain Constraints

Due to industry-wide shortages of specific contrast agents and injectables, the following procedures have limited availability at outpatient centers at this time: Epidural Steroid Injection (ESI), Fine Needle Aspiration (FNA), and Myelography. We are actively monitoring this situation and will resume full availability as soon as materials allow. Arthrograms remain fully available. We appreciate your patience and understanding.
